Terms and conditions of stay
NOTICE TO PET OWNERS
Please read the following terms and conditions carefully before checking in your pet.
I, _______________________________________, owner of the pet named _______________________________________, hereby confirm and certify that the pet entrusted to Purr Paws Cat Hotel is in good health and free from any illness or medical condition. I further confirm that I have provided valid health examination records and vaccination certificates issued and certified by a licensed veterinarian from a veterinary clinic or animal hospital.
I hereby declare that my pet is not suffering from any illness, abnormal condition, contagious disease, or chronic medical condition requiring specialized care, close supervision, or treatment by a veterinarian or veterinary nurse. I acknowledge that Purr Paws Cat Hotel does not employ an on-site veterinarian or veterinary nurse capable of providing specialized medical care.
Should any injury, illness, deterioration of health, or death occur as a result of undisclosed health conditions, incomplete information, or concealment of material facts by the pet owner, Purr Paws Cat Hotel shall not be held liable or responsible for any resulting loss, damage, injury, medical expense, or death under any circumstances whatsoever.
In the event that the pet develops any unexpected illness, injury, or medical emergency during its stay due to unforeseen circumstances, Purr Paws Cat Hotel will first attempt to contact the owner using the telephone number or contact information provided. Upon obtaining the owner's consent, the hotel may arrange transportation of the pet to the nearest veterinary hospital, including but not limited to Piyavate Animal Hospital or Kaset Sritrang Animal Hospital. All expenses incurred shall be charged at actual cost and must be reimbursed by the owner upon collection of the pet.
If the owner cannot be reached in a timely manner during an emergency situation, the owner hereby authorizes Purr Paws Cat Hotel, in advance, to immediately transport the pet to the nearest available veterinary hospital without prior approval, for the sole purpose of protecting the pet's health and safety. The owner agrees to be fully responsible for and promptly reimburse all medical, transportation, and related expenses incurred as a result of such emergency treatment.
If, after the scheduled pick-up date specified in the check-in documentation, Purr Paws Cat Hotel is unable to contact the owner through the telephone number, Line account, or any other contact method provided for a period exceeding forty-eight (48) hours without prior notice or reasonable explanation, the hotel reserves the right to pursue legal action and any other remedies available under applicable law. The owner acknowledges and agrees to such action without objection.
As previously communicated in the boarding requirements of Purr Paws Cat Hotel, pet owners are responsible for providing all necessary supplies for their pets during their stay, including but not limited to cat litter, food, treats, toys, and any other essential items. Should any such supplies be depleted during the boarding period, the hotel will first notify the owner and obtain approval before purchasing replacement supplies on the owner's behalf. The owner agrees to reimburse the hotel for the actual amount paid in advance for such purchases upon collection of the pet.
(If you are unable to provide your pet's regular cat litter, the hotel offers tofu cat litter at a service charge of THB 39 per day.)
I hereby acknowledge that I have carefully read, fully understood, and voluntarily accepted all terms, conditions, responsibilities, authorizations, and obligations set forth in this agreement. I further certify that all information provided by me is true, accurate, and complete, and I agree to comply with all provisions contained in this document and its accompanying pages.
